Dorsalis Pedis Flap


Abstract
Treatment of soft-tissue defects of the extremity has always been a challenging problem to patients and surgeons. There are many pedicular or free flaps currently being used in reconstruction of extremities.
We analysed the 19 cases of dorsalis pedis flap which were used for the coverage of the soft tissue defect of the extremity since 1988. The cases were followed at least 1 year and an average of 3 years and 1 month.
@ES The results were as follows:
@EN 1. The average size of flaps was 9¡¿6cm(maximum 11¡¿8, minimum 7¡¿5cm).
2. The types of the flaps were free flaps in 14 cases(74%) and island flaps in 5 cases(26%). Among them, there were 6 sensible flaps and 2 tendocutaneous flaps.
3. The causes of the soft tissue defect were trauma in 11 cases(58%), osteomyelitis in 6 cases(32%), deformity correction in 1 case(5%) and pressure sore in 1 case(5%).
4. The recipient sites(soft tissue defect sites) were dorsum of the foot in 7 cases(37%), heel in 5 cases(32%), ankle and distal tibia in 4 cases(22%), dorsum of the hand in 2 cases(10%) and plantar surface of the foot in case(5%).
5. The types of the first dorsal metatarsal artery of the donor site were IA in 11 cases(58%), IB in 6 cases(32%) and IIA in 2 cases(10%).
6. The flaps were taken firmly in all of the cases. There were several complications during follow up period which were delayed wound healing in 4 cases(21%) and hypertrophic scar formation in 3 cases(16%).
7. Pain and tactile sensibility of the recipient site were noted in all cases, but the two-point discrimination was noted in only island flaps(average 12mm).
